If your post incident review is focused on remediation for machines and not on people, it's no better than retributive justice.

Machines don't matter. People do, as they're the ones keeping systems up.

If we want software engineering to be a discipline, we need to understand safety in the same way other industries (see: aviation, medical, etc.) and not just "well, we'll deploy this code and see what breaks!"

Realization in my 30's: giving 100%, maybe even giving 90%, to anything for an extended period of time is detrimental to whatever you're putting yourself into.
